New Patient Visit

New patient visits can be completed in person or done virtually. Please note that pre screening and pre appointment paperwork is required. Contact us directly to schedule your appointment and have your paperwork either delivered or emailed. A self addressed envelope will be included to return your completed paperwork, or you have the option to complete the forms online through our secure portal.

Follow up or Sick Visit

Follow up and sick care visits can be done through telehealth or in office. Sick appointments can usually be seen the same day and follow up visits are scheduled within the same week.
